How Small Exceptions Become the New Normal
Normalisation of deviance rarely announces itself. It starts with a single justified shortcut under time pressure, nothing goes wrong, and the shortcut quietly becomes the way the job is done — until the procedure on paper and the procedure in practice have drifted apart without anyone deciding that they should.
Mature facilities are especially exposed because experienced crews have often seen the 'wrong' way work safely dozens of times, which erodes the perceived risk faster than any training program can reinforce it.
Catching Drift Before It Compounds
Behavioral audits that focus on why a deviation happened, rather than simply logging that it did, tend to surface the underlying pressure — understaffing, unrealistic timing, unclear ownership — long before it produces a serious incident.
Leadership visibility matters more than paperwork here: when supervisors are seen enforcing the standard consistently, deviation is far less likely to quietly become policy.
