Bridging Qualitative and Quantitative Risk
LOPA earned its place in the process safety toolkit by solving a specific problem: HAZOP identifies scenarios well but is a poor tool for deciding how much protection is actually enough. LOPA adds order-of-magnitude rigor without the full cost and time of a quantitative risk assessment.
The method's value depends entirely on the discipline applied to independent protection layer credit — claiming too much overlapping credit for layers that share a common failure mode is the most common way LOPA studies quietly understate real risk.
Common Pitfalls in Practice
Teams under schedule pressure often reuse generic initiating event frequencies without checking whether site-specific data suggests something different, which can materially skew the resulting risk ranking.
A well-run LOPA session treats every claimed protection layer as needing to earn its credit through independence, effectiveness, and auditability — not simply because it appears on the P&ID.
